Your First Visit
Once you have been checked in by our friendly front desk, a member of our team will escort you to the back and take some initial X-rays and photos of your mouth. You’ll then get to meet your dentist, who will spend a few minutes chatting with you before performing an exam. Afterward, they’ll share their observations with you, go over the images from earlier, and recommend any additional treatments you might need. When all of your questions have been answered, you’ll be brought back to the front desk to schedule your next appointment.
New Patient Forms
You now have the option to fill out our patient forms in the comfort of your own home. Just click on the link below to download them. Complete them at your leisure, and be sure to bring the forms with you on the day of your appointment. This way, we can get you checked in nice and fast and virtually eliminate any wait time. Dental Insurance Welcome
At Cross Creek Dental Care, our insurance policy is simple: if you have dental insurance, we’ll accept it! Not only that, but we’ll also handle the claims process for you so we can help you maximize your benefits without having to fill out all that paperwork. Our team is very familiar with the ins and outs of various plans, so if you have any questions about using your particular policy with us, don’t hesitate to give us a call.

If you don’t have dental insurance, our Dental Savings Plan offers an excellent option to help you save on your care every time you come to see us. For just one low flat fee, you can completely cover your basic dental care for the entire year as well as receive a 15% discount on all other treatments (including cosmetic procedures that are typically not covered by insurance).
